In Public Noncommercial Utility Company “Lviv City Polyclinic №3” at the address Povitriana str., 99 (Levandivka district) one can get paid medical services, besides there is a commission on occasional medical examinations for workers (under the order №246 of MOH of Ukraine from 21.05.2007) and medical check-ups for receiving “Individual medical record book” (the order №280 of MOH of Ukraine from 23.07.2002).

The price of basic medical check-up is 578.00 hryvnias (including VAT). Moreover, we organize additional medical examinations due to harmful working conditions and in accordance with the list of professions, as well as medical check-ups for receiving “Individual medical record book” for 400 hryvnias (including VAT).

The staff of the polyclinic will ensure qualified medical services, as our institution has all the necessary equipment, instruments and materials for carrying out medical examinations.
